Q:  What should I bring with me to my first visit?
A:  New patients should bring a picture ID (driver's license or State ID), Proof of Residence (utility bill, driver's license, piece of mail with your name and address), Social Security Card, most recent Tax Return (1040 or 1040EZ), and a list of current medications or medication containers.

Q:  Is there anywhere I can go for a free mammogram if I am 40 years old or older?
A:  Patients who meet certain requirements can contact the Premier Community Health Program at 866-838-8973 or the Women's Wellness Fund at 937-299-0099.
